The following is a listing of magazines that are available in our fundraiser. There are 3 ways to order:

1. Send in an order form. In order to do this, you must print out the order form, make sure you fill it out completely with the exception of item numbers, and mail it to:

This needs to be done as soon as possible, as I must have all orders by Monday, Dec. 02, 2002. Any orders recieved after that time will be returned, and we will not be able to process it.

2. Call in an order. I will have my cel phone on me for the duration of the fundraiser, and I can be reached at (859) 200-2474. I will be happy to fill out the order form for you. If I do not answer, just leave me a message and I will get back to you as soon as possible.

3. Fill out the order form below. If you would like to fill out the order form below, that is fine, but I cannot guarantee security for credit card information, so you will have to call me, (859)200-2474 if you wish to use a credit card, or mail the check to the above address if you choose to write a check. Remember, if you choose the check payment, we must recieve payment no later than 12/02/02, or we will not be able to process the order.
